Here’s some reasons why many people often don’t do what they plan for, in my experience:

They try to do too much (one area I’ve had to learn a lot in myself)

They don’t set clear and definite priorities.

They spend too much time online and on email

They go to meetings they shouldn’t go to and waste too much time at meetings in general
